When your primary care provider on-Base refers you for specialty care, we’re here to secure specialty appointments and diagnostic testing, in a timely manner — within 48 hours for non-routine care.

How it Works

Visit your primary care physician at the 87th Medical Group who will provide any needed referrals for specialty appointments and/or diagnostic testing. Then call 1-866-9-HERO-CARE (1-866-943-7622) to connect with a dedicated HeroCare Connect™ Patient Service Specialist and Navigator who will do the rest— taking care of every appointment detail, working with Humana/TRICARE and the Base, making the process smooth and seamless for you.

Community Care Network (CCN)

How it Works

If you are enrolled in the Community Care Network (CCN), visit your primary care physician at the VA. If your primary care physician recommends a specialist and/or diagnostic testing and the VA does not offer that service or specialty, or if they cannot secure an appointment for you within 28 days, at a geographically convenient location for you, have your VA care coordinator or provider call 1-866-9-HERO-CARE (1-866-943-7622). A dedicated HeroCare Connect Patient Service Specialist and Navigator will do the rest — taking care of every appointment detail, working with the VA and TRICARE, making the process smooth and seamless for you.

Private Insurance/Medicare

military familyIf you have private insurance or Medicare, call 1-866-9-HERO-CARE (1-866-943-7622) to connect with a dedicated HeroCare Connect Patient Service Specialist and Navigator who will do the rest—taking care of every appointment detail, working with your insurer to make the process smooth and seamless for you.

If you are not part of the Community Care Network and receive care from the VA, you can also call 1-866-9-HERO-CARE (1-866-943-7622). Our HeroCare Connect Patient Service Specialist and Navigators are very knowledgeable about the VA and various insurance products and coverages. Our team has a close working relationship with the region’s VA clinics and hospitals.
